BHR’s services are without charge to individuals who receive the services.
Client Rights:
You have the right to:
- Request any service and receive prompt evaluation, care, referral, and confidential treatment for which you are eligible.
- To have the same legal rights as any other citizen.
- Be actively involved in your treatment.
- Be informed of your treatment in understandable language and have access to your records.
- Receive any services you need in a convenient manner.
- To refuse hazardous treatment and to be the subject of experiment only with signed consent.
- Request a fair hearing and be heard.
- Be treated with respect and dignity and be free from verbal, physical, and fiduciary abuse.
- Not be denied admission or services because of race, creed, marital status, national origin, handicap, or age.
- Be informed of rights in understandable language.
Client Responsibilities:
In order to provide you with the best possible care, we need you to follow these rules:
- Make sure you provide us with complete and accurate information.
- If you do not understand, please ask.
- If you must cancel a Next Day Urgent appointment, then you must call the Center where you are to receive services the day of the appointment.
BHR’s Responsibilities:
At BHR we believe that no one deserves to receive any kind of neglect or abuse. Therefore, if you have a complaint of any kind, report it to a counselor, a supervisor, and/or the Vice President of Operations at BHR. An investigation of your complaint will be initiated immediately. If you believe any our your posted rights have been violated, you may also file a grievance with the same representatives.
